When you arrive for your first visit, you will be welcomed by our courteous staff. In order to create your medical chart, our staff requires your state ID or driver’s license as well as your current medical insurance card. If you have not completed and brought your New Patient Registration forms prior to your visit, you will be asked to complete them in the office prior to being escorted to the exam room.
Please bring any current medications with you to your first visit so that Dr. Mikhail can review them and address any questions. If you have any copies of recent medical records from the last 12 months, please bring them as well. Those records will help Dr. Mikhail to document your medical history and allow him to get to know you in order to provide the best comprehensive care for you.
